Here are some explanations on what to fill in and where:

sigpic/userbar/avatar: just fill in weather you want a sigpic, avatar or userbar. sigpic are the big things, userbars are the small bars, avatars the square pictures under your name.
background: what do you want for a background. for userbars it must be a gradient, for sigpics it can be anything: ill do my best.
Image(s): what for image do you want on it, multiple are possible.
Text: What so you want the text to say?
Font(opt.): what font do you want us to use? [/color]
Text color (userbar only): what color do you want the text to be? normally white.
Text Border color(userbar only): the little line around the text, normally black.
Attach with (optional, userbar only): do you want another userbar attached to it? tell it here.
Animate with attached? (optional, userbar only): so you want the attached userbar to fade, slide or whatever animated effect? put it here. NOTE: please fill out a 2nd form for the attached one.
Animation (optional, sigpic and avatar only): do you want anything moving in your sigpicor avatar, ill try my best. (note that i cannot do any picture moving, but just fades or layer moving)
